Maine Veterans' Homes Physical Therapist Per Diem in Bangor, Maine
Job Related Responsibilities and Activities:
Evaluates resident and develops effective treatment plan.
Ensures that appropriate care is being provided by reviewing residents’ Care Plan and residents’ chart for specific treatments, medication orders, changes in condition, etc.
Completes required documentation including daily notes, weekly progress notes, discharge summaries, etc. utilizing point of service documentation as indicated.
Prioritizes and manages time to meet the MVH department productivity standard of 80%. This is direct billable time only divided by time in facility.
Participates in discharge planning..
Orients and trains new personnel as directed by Rehabilitation Manager.
Conducts PTA and RA reviews in a timely manner and recommends merit/bonus awards.
Performs duties in a manner that assures maximum safety to self and others.
Works weekends/holidays to provide necessary physical therapy coverage as needed.
Assists Rehabilitation Manager in the development of new programs and services.
Conducts training programs for facility staff in conjunction with the Staff Development Coordinator.
Attends IDT meetings, Rehabilitation Staff meetings, and Management Staff meetings as necessary regarding problems, progress, and needs of the residents.
Complies with Rehabilitation policies and procedures, State and Federal guidelines, and VA regulations.
Displays respectful behavior to residents, families, and visitors, works professionally with vendors and others outside MVH, and provide positive information to others on MVH services.
Maintains close working relationship with the Physical Therapy Assistants, Occupational Therapists, Occupational Therapy Assistants, Speech Therapists, and Rehab Aides.
Works cooperatively with staff in all other departments.
Represents MVH locally, state-wide, and nationally through committee appointments, board memberships, professional organizations, and other activities.
Attends various facility committee meetings as requested by Rehabilitation Manager.
Ensures residents’ rights are upheld by Rehabilitation staff.
Ensures therapy equipment safety, cleanliness, and function.
Reports all work-related problems, violations of resident’s rights, unusual events, unethical behavior, criminal activity, complaints/grievances, and other work-related concerns to the Rehabilitation Manager before the end of his/her shift.
Attends annual educational sessions on fire safety, infection control, disaster, hazardous communication, dementia related issues, resident rights, and other topics as assigned.
Maintain current licensure and submits renewed license prior to expiration to HR.
Keeps informed on physical therapy trends, resources, procedures, and community resources; updates department accordingly.
MVH employees must promptly report, to one's supervisor, Human Resources or through the Hotline, any activity that the employees believe in good faith may be a violation of a law, regulation, MVH policy or the MVH Code of Conduct. In addition, each employee must cooperate with any investigation conducted or sponsored by MVH in connection with any alleged violation of policy, law, regulation or code.
Perform other duties as assigned by Rehabilitation Manager.
Rehab Manager may assign resident focused activities to individuals within the therapy department that is NOT calculated in the productivity for that day. This additional resident focused activity is based on resident needs for that day and will be considered separate from the expectations of the department as outlined in the specific job descriptions.
Graduate of an accredited Physical Therapy program. Possesses a minimum of a Bachelor’s Degree in Physical Therapy. Must have current State of Maine Physical Therapist license. Experience in long-term care environment preferred.
