Beaumont Health System Physical Therapist Spec in Farmington Hills, Michigan

Outpatient Experience required. Manual Therapy & Vestibular Rehab certification preferred GENERAL SUMMARY:The physical therapist utilizes her/his professional mobility and locomotion expertise to evaluate the anatomic structures and physiologic functions that interact to move the body or its component parts and provide customized interventions and plans of care to optimize functional capacity. The therapist focuses on obtaining the individual's participation to achieve their desired activity level leading to improvement in their quality of life. It is also necessary for the physical therapist to provide timely medically necessary documentation supporting skilled care and appropriate communication along with interaction with other medical professionals as necessary to attain the individual's therapy goals. Their performance is in collaboration with supervisory support and according to Beaumont Health's Mission, Vision and Values as well as Corporate and Department policies and procedures. In addition to the primary duties and responsibilities, uses appropriate certification to perform comprehensive lymphedema/venous insufficiency management programs as prescribed, pelvic floor rehabilitation, neuro-developmental techniques, Osteopathic manipulative physical therapy (OMPT), LSVT and other non-par treatment protocols as prescribed. Recognition of a specialty designation is made only with department management approval.ESSENTIAL DUTIES:1. Provides thorough and appropriate initial evaluations and timely re-evaluations of the patient's physical ability and any underlying impairments that affects the patient's function and quality of life consistent with physician referral, as needed, incorporating a review of the patient's medical history and records.2. Creates customized plans of care that coordinate therapist and patient directed goals to obtain achievable outcomes for prevention, intervention and sustainable wellness over the patient's lifespan.3. Implements and progresses evidence-based therapy interventions consistent with established and modified goals and plans of care according to the patient's response to treatment appropriately utilizing physical therapist assistant and technical aide support. Develops and coordinates with the intra/inter/multidisciplinary team an integrated discharge plan and recommendations specific to the patient's needs.4. Addresses any patient barriers to understanding or learning and involves the patient, family and/or caregiver in the entire therapy experience per the patient's preference. In addition, collaborates and communicates with other healthcare team members to promote optimal benefit from therapy and quality care.5. Performs medically necessary documentation consisting of initial evaluations, re-evaluations, daily notes, plans of care with associated certification dates, physician letters, insurance authorizations and discharge notes/discharges which provide evidence of skilled care in accordance with the department's delineated timeframes.6. Fulfills administrative duties associated with patient care including scheduling assistance, follow through with patient attendance, accurate and timely charging practices, assurance of appropriate referral and diagnostic coding, utilization of only the medically necessary number of visits, compliance with insurance authorizations and tracking of insurance limitations, participation in peer reviews, performance and process improvement projects, program development, community events and any other related activities.7. Meets department productivity metric expectations, i.e., average visits per day, average skilled units per visit per day, average cancellation rate, average schedule fill rate, etc. to promote the financial success of Beaumont Health.8.
