Our unique workplace Culture of Excellence is built upon: *Employee engagement, empowerment and growth *Teamwork toward our common goal - providing exceptional health care services with a passion for making lives better *An atmosphere of caring and quality that cascades throughout the organization Department/Skill highlights *Janesville, Behavioral Health Outpatient *Monday - Friday Essential duties and Responsibilities *Investigates case situations and presents information to Psychiatrist and other members of health team, on patient's family and social background pertinent to diagnosis and treatment. *Helps patients to respond constructively to treatment and assist in adjustment leading to and following discharge. *Interprets psychiatric treatment to patient's family and helps to reduce fear and other attitudes obstructing acceptance of psychiatric care and continuation of treatment. *Serves as link between patient, psychiatric agency, and community. *Works directly in treatment relationship with patients, individually or in groups, in consultation with Psychiatrist. *Works with patients who present with AODA and/or Mental Health concerns. *Counsels clients individually and/or in group sessions to assist client with their mental health issues and/or in overcoming alcohol and drug dependence through monitoring the client (s) treatment success and adapting treatment as needed. *Completes psychosocial assessments, treatment plans, and ongoing treatment with individuals, groups, and families in cooperation with patient and multidisciplinary team. *Delivers behavioral health care in accordance with patient evaluation, treatment plan, and within the scope of their license. *Consults and coordinates care of patient with physician, program coordinator and agencies. *Assesses treatment planning and progress. *Provides crisis, assessments and intervention. *Maintains pertinent patient data in medical record in accordance with state, JCAHO and Mercy Health System guidelines. *Offers in-service education programs and functions as a resource to other staff and the community as needed. *Participates in development and selection of patient education materials and services. *Participates in performance improvement activities. Education and Experience Master's Degree in Counseling, Social Work, Psychology or other related field Certifications/Licensure Licensed to independently provide both mental health and substance abuse treatment in Wisconsin.
