McLaren Health Care Physical Therapist in Lansing, Michigan
The physical therapist will accurately assess,
differentially diagnosis, and identify deficits in a patient’s ability to move
and perform functional activities in their daily lives. They develop an individualized treatment plan
with patient-centered goals to reduce pain, restore function, and prevent
disability and work with the individual to prevent the loss of mobility.
Essential Functions and Responsibilities :
Performspatient evaluations through the process of obtaining history, performingrelevant systems review, and selecting and administering specific tests andmeasures to obtain data.
Establishesan individualized plan of care and selects treatment techniques that addressesthe specific impairments identified in the evaluation and in accordance withphysician orders.
Conductsongoing reassessment of the patient and their response to treatment.
Initiatesand manages the discharge plan beginning at the first visit and continuingthroughout treatment.
Supervisesand delegates to ancillary staff and students consistent with state laws anddepartment policy.
Completeslegible, accurate and clear documentation within the established timeframeidentified in the department policy and procedure and in accordance withaccreditation requirements and insurance regulations.
Communicateswith the patient, family, caregivers, significant others, and members of thehealth care team to promote maximum benefit of care utilizing AIDET.
Assesseseducational needs of the patient, family, caregiver, or significant other andprovides education to meet those needs.
Participatesin professional growth and development through participation in educationalprograms, including in-service meetings, workshops, and by reading currentliterature concerning scope of practice.
Activelyparticipates in quality improvement activities.
Provides anenvironment conducive to safety for patients, visitors, and staff.Complies with appropriate and approved safetystandards.
Functionswithin scope of practice as defined by the state’s practice act.
Performs other related duties as required anddirected.
Graduate of an accredited program of physical therapy.
Current state Board of Physical Therapy licensure.
Preferred: One year of physical therapist experience.
