This position serves as Registered Respiratory Therapist (RRT) for inpatient and outpatient services. The RRT administers various respiratory therapy procedures, assesses the respiratory status of patients utilizing physical observation and clinical analysis to determine therapy effectiveness and makes recommendations to providers regarding treatment changes. The incumbent is responsible for his/her individual program within the service. The services are performed in accordance with the guidelines established by the National Board of Respiratory Care: (NBRC) WI licensing Board and the American Association for Respiratory Care (AARC), and policies and procedures of Middleton VA Hospital. The protocol and procedures are established for the treatment, management, evaluation ,and care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. The incumbent will be competent to perform within all areas of the section to meet patient care needs. Major Duties: Demonstrates knowledge and level of expertise required to perform Respiratory Care and Diagnostic services to care for patients. Monitors and evaluates patient performance during procedures and evaluates' response to therapy and recommends changes based on information obtained. Demonstrates proficiency at obtaining and delivering acceptable anaerobic arterial blood gas samples. Interacts effectively with patients, physicians and other members of the healthcare team. Consistently communicates and treats customers (veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner. Maintains confidentiality of all sensitive patient and employee records and information, and limits disclosure of such information to only those individuals who have a specific job-related need to know the information. Competence maintained during the evaluation period according to departmental and unit specific standards, including in service and continuing education. Demonstrates understanding of the range of interventions needed by patients according to their age, developmental level and physical condition. The ability to interpret and integrate information in terms of the patient needs based on age, developmental level and physical condition. Demonstrates interpersonal skills necessary to communicate effectively and positively with a wide variety of internal and external customers. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: RRTs appointed to direct patient-care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education. Individuals must have successfully completed a respiratory care program accredited by the CoARC or its successor. Licensure. Persons appointed or reassigned to RRT positions in the GS-0601 series must possess and maintain for the duration of employment a full, current and unrestricted license from a state to practice as an RRT. Credential. Persons appointed or reassigned to RRT positions in the GS-0601 series must possess and maintain for the duration of employment a valid, current, unrestricted credential of RRT. Grandfathering Provision: All persons employed in VHA in this occupational series or in another occupational series and performing the duties as described in the qualification standard on the effective date of this qualification standard are considered to have met all qualification requirements for the grade held, including positive education, credential or certification that are part of the basic requirements of the occupation. Grade Determinations: GS-05: Experience. None beyond the basic requirements. (b) Education and Licensure. This level assignment is designated as temporary under paragraph 4d(1) above for individuals who have successfully completed a respiratory care program accredited by the CoARC or its successor, acquired the CRT credential, licensed by their state and are working towards obtaining RRT credential. GS-7: Experience, Education, Licensure and Registration. In addition to the basic requirements, candidates must possess their RRT credential. In addition to the requirements above, candidates must demonstrate all the following KSAs: 1. Ability to perform therapeutic modalities related to respiratory care. 2. Skill in analyzing physiological specimens obtained to include arterial blood gases, pulse oximetry, End tidal Co2 (ETCo2) monitoring, basic spirometry and other basic diagnostic procedures. 3. Ability to collaboratively consult with physicians and other healthcare professionals for respiratory care to help provide favorable patient outcomes. 4. Skill in assessing oxygenation and ventilation. 5. Ability to perform basic airway management to include artificial airways and mechanical ventilation. GS-9 Experience or Education, Licensure and Credential. Candidates must have: One year of creditable experience equivalent to the GS-07 grade level demonstrating the clinical competencies described at that level. OR Master of Science degree in respiratory care AND License AND ARRT credential. In addition to the requirements above, candidates must demonstrate all the following KSAs: 1. Skill in performing therapeutic modalities related to respiratory care. 2. Ability to collaboratively consult with physicians and other healthcare professionals to help provide favorable patient outcomes. 3. Skill in assessing oxygenation and ventilation. 4. Ability to perform airway management to include artificial airways and mechanical ventilation. GS-11. Experience, Licensure and Credential. Candidates must have: One year of creditable experience equivalent to the GS-09 grade level demonstrating the clinical competencies described at that level. Experience includes: obtain, interpret and analyze physiological specimens and data. They consult with physicians and other healthcare professionals to ensure quality of patient care within area of specialty. Manage and maintain the airway and ventilation of the patient through the use of appropriate mechanical means. Assess and evaluate patients to determine and recommend intervention to develop and implement a plan of care, such as, implementing protocols, intubation, monitoring critical systems and uses advanced modes of ventilation. Provide outpatient services such as home ventilators, PAP therapy, oxygen therapy and COPD care. AND Hold an active license and RRT credential. In addition to the requirements above, candidates must demonstrate all the following KSAs: 1. Ability to collaboratively consult with physicians and other healthcare professionals for complex respiratory care to develop care plans to provide favorable patient outcomes. 2. Skill in evaluating the efficacies of therapeutic modalities. 3. Skill in performing advanced airway and ventilation management. References: VA Handbook 5005, Part II, Appendix G11.
