Integrates the occupational therapy treatment planinto Interdisciplinary Team plan of care. Assists in the ordering,inventory, distribution and maintenance of durable medical equipment forparticESSENTIAL FUNCTIONS:1. Conducts an in-person initial comprehensive occupational therapy assessmenton all participants upon enrollment and coordinates with the InterdisciplinaryTeam on a Plan of Care for each participant.2. Conducts an in-person re-assessment minimally every six (6) months oras needed.3. Provides evaluation/treatment on acute, restorative and maintenancelevels to participants.4. Functions as a member of the Interdisciplinary Team. Maintains regularattendance at, and participates in Interdisciplinary Team meetings;communicates participant changes, collaborates on plan of care decisions andcoordination for twenty-four (24) hour care delivery.5. Administers treatment program per physician instructions. Clarifiesphysician's treatment instructions. Confers with Interdisciplinary Team onparticipants care issues.6. Evaluates participant's needs. Follows-up with participants duringtreatment, explaining precautions to take. Discusses condition and treatmentplans and recommends solutions to participants care problems withInterdisciplinary Team.7. Carries out accepted principles and practices of occupational therapy forprevention of functional decline, sensory and cognitive limitations,increase or maintenance of strength and endurance, and provides treatment toincrease self-care skills.8. Constructs or provides orthotic and adaptive equipment as needed.9. Conducts assessment and coordination of home modification program includinginstallation of equipment and other changes to support independence andsafety.10. Assists as liaison with nursing home and assisted living staff regardingoccupational therapy needs of participants who are their residents.11. Evaluates charts and reports on participant's progress, documentsoccupational therapy notes in the participant's medical record. Fulfillscharting and reporting requirements.12.
