Provides clinical physical therapyevaluation and treatment for program participants, on acute, restorativeor maintenance levels as needed by Day Center, Nursing Home or in-homesetting. Integrates the physical therapy treatment plan into theInterdisciplinary Team plan of care. Assists in the ordering, inventory,distribution and maintenance of durable medical equipment for participantsESSENTIAL FUNCTIONS:1. Conducts an in-person initial comprehensive physical therapy assessment onall participants upon enrollment. Coordinates with the Interdisciplinary Teamto develop a comprehensive plan of care for each participant.2. Conducts an in-person physical therapy re-assessment minimally every six(6) months or as needed.3. Functions as a member of the Interdisciplinary Team. Maintains regularattendance at, and participates in, Interdisciplinary Team meetings;communicates participant changes, collaborates on plan of care decisions andcoordination for twenty-four (24) hour care delivery.4. Provides treatment to participants and implements physical therapyprocedures consistent in achieving identified goals. Grades activities tofacilitate goals. Confers with Interdisciplinary Team on participants careissues.5. Evaluates participant's needs. Follows-up with participants duringtreatment, explaining precautions to take. Discusses condition and treatmentplans and recommends solutions to participants care problems withInterdisciplinary Team.6. Assesses the need for adaptive Durable Medical Equipment (DME),specialized shoes and prosthesis followed by recommendations toInterdisciplinary Team. Receives authorization prior to purchases. Assessesall newly issued orthotic shoes, braces and prosthetics for appropriatenessand proper fit.7. Assists as liaison with Senior CommUnity Care nursing home and assistedliving staff regarding physical therapy needs of participants who are theirresidents.8. Evaluates charts and reports on participant's progress, documentsphysical therapy notes in the participant's medical record according toSenior CommUnity Care charting requirements.9. Assists in recommending physical therapy policies and procedures concerningservices provided. Maintains quality assurance program, safety,environmental and infection control.10. Directs, supervises, performs initial and annual competencies, andevaluates the performance of the Physical Therapy Assistant.11. Performs competencies on appropriate direct care staff as needed.12. Ensures that the job responsibilities, authorities, and accountabilityof direct reports are defined and understood.13. Assists in training and provides ongoing education and clinical
