Our services include short-term rehabilitation, long-term care, skilled nursing, hospice care and respite care. Our short term rehabilitation unit features private suites with private baths. Therapy services are provided in a well-appointed therapy gym equipped with the tools necessary to reestablish an independent lifestyle. Our facility also features a peaceful chapel and vibrant activities programming.: Orchard Villa, a 136 bed skilled nursing facility conveniently located near I-280 and Navarre Avenue, offers a warm and comfortable environment with staff committed to treating everyone like family- with compassion, respect and kindness. Orchard Villa has been a value part of the Oregon community for over 20 years. The facility offers almost exclusively private rooms, physician led specialty programs and an on-site hemodialysis unit in collaboration with Dialyze Direct: Summary: The Occupational Therapist will provide evaluation and treatment to residents of the skilled nursing facility as dictated by the American Occupational Therapy Association, given physician orders. You will communicate with families, physicians and other team members verbally and in the medical record and provides staff training and ensure carryover of skilled therapy goals.: Education and Experience: * Bachelor of Science/Arts in the field of Occupational Therapy. * Registered and licensed therapist in good standing in the Occupational Therapy field. * At least one year experience in the practice of Occupational therapy. * Direct care experience required – minimum one year Why Legacy Health Services?
