Presbyterian Homes and Services Casual Physical Therapist & Physical Therapist Assistant in Plymouth, Minnesota

Presbyterian Homes & Services is seeking Casual Physical Therapists & Physical Therapist Assistants to join our Rehab team in the West Metro. Specifically, we are looking for therapists interested in supporting the following communities:

  • Interlude West Health, Plymouth, MN (weekday & every other weekend needs)

  • Flagstone, Eden Prairie, MN (this is a growing campus with opportunity for part/full time in the future)

  • Maranatha, Brooklyn Center, MN

  • Lake Minnetonka Shores, Spring Park, MN

  • Pres Homes of Bloomington, Bloomington, MN

As an on-call therapist serving at PHS, you are hired and onboarded at one of our locations and report directly to that Rehab Program Manager. After you have successfully completed new hire orientation, you may pick up shifts at other communities. PHS Rehab on-call requirements are a minimum of one shift per month. Preference to candidates who have weekday and weekend availability and can support around holidays.

The Physical Therapist is responsible for planning, developing, and implementing and monitoring high quality physical therapy care and services to residents consistent with regulations and established rehabilitation policies, procedures and best practices.

• Provide direct care of residents with physician ordered evaluation and treatment based upon regulations and established rehabilitative procedures and best practices.

• Provide consultation, evaluation, and assessment of residents as needed. Implement treatment plans.

• Participate in patient/resident care planning with the team and other health care provider.

• Provide patient/resident and family education/consultation.

• Communicate resident status and needs to the patient/resident, their family, staff and other professionals (i.e. care conferences). Refer to other professionals as appropriate.

• Participate in discharge planning processes.

• Maintain records by practice standards, third party payers and regulatory agencies.

• Complete necessary portions of MDS/QMDS/care plans as required.

• Maintain all required documentation in accordance with regulations and established rehabilitation best practices.

  • Graduate from an accredited Physical Therapy program.

  • Current licensure in the state of practice as required

  • Competence in Physical therapy evaluation and treatment.

  • Demonstrated compatibility with PHS's mission and operating philosophies.

  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
