Genesis Healthcare Clinical Therapist (Full Time) in Pomona, California

The Clinical Therapist assists in the planning and implementation of treatment for patients in a psychiatric facility. The therapist is an integral part of the interdisciplinary team and provides individual therapy, family therapy, group therapy, psychoeducational groups, and consultation to treatment staff. The therapist utilizes their knowledge and skill set to assist residents with non-pharmacological interventions, solution focused problem-solving, and comprehensive treatment utilizing research based methods. Some of the additional job responsibilities are as follows:

Prepares a comprehensive written psychosocial evaluation and assessment on assigned residents, in addition to an intake assessment, care plans, regular progress notes, court reports/letters, and a discharge summary.

Responsible for discharge planning including: assessing a resident's readiness for discharge, and coordination planning with the treatment team; making appropriate referrals for placement, case management and aftercare services; coordinates with Social Services; and maintains a knowledge of community resources.

Communicates to residents their legal status and rights under applicable law.

Provides consultation as needed with families and Social Services on issues regarding admission, discharge, and aftercare plans.
