As a Physical Therapist (PT) at Family Home Health Services you will:
Provide therapy services for each patient based on their plan of care, within your scope of practice, in the patient’s home.
Complete comprehensive assessments incorporating the OASIS data set, as required by Medicare, utilizing OASIS information to assist in the development of the plan of care.
Assists the physician in evaluating the patient’s level of function and helps develop and revise the plan of care.
Evaluate patient for appropriateness of admission and verify the patient meets homebound status.
Educate the patient and family members on treatment plan, goals and expected outcomes.
Prepare and submit clinical and progress notes, reflecting activities, findings and patient’s reaction to treatment and changes to patient’s condition in a timely manner following visit or communication.
Attend virtual staff meetings and participates in patient case conferences/coordination of care communication.
Other duties as assigned.
Graduate of an accredited Physical Therapy program.
Licensed as a Physical Therapist in Florida.
At least one year of experience treating adult patients, preferably geriatric patients.
