Providence is calling a Therapist Enterostomal (RN) - Home Health West Portland (1.0 FTE, Days Full-Time) to Providence Home Services Oregon in Portland, OR

We are seeking a Therapist Enterostomal (RN) - Home Health West Portland that will be accountable for direct and indirect quality care for patients of all ages, according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. Accepts professional development as an integral part of nursing practice.

The ET will function as an educator and consultant to the Home Care staff, patients, and in the community, as needed. The ET will coordinate patient care activities with other members of the health care team in a collegial relationship.

MAJOR CHALLENGES:

  1. Managing a nursing practice that includes patients requiring a broad spectrum of care, who reside in a variety of environments, in a wide geographical area, by practicing good organizational and time management skills.

  2. Recognizing and responding immediately and appropriately to potential emergency situations that arise.

  3. Maintaining patient clinical records in accordance with regulatory standards.

  4. Maintaining and enhancing the quality improvement (QA/QI) of patient care.

  5. Maintaining a high level of creativity and leadership within a rapidly changing health care environment.

In this position you will have the following responsibilities:

  • Utilizing the nursing process, assess patient status, intervene appropriately, and evaluates patient response

  • Establish a formal written plan of care for referred patients, documents specific nursing orders, and develop goals specifying expected outcomes of intervention

  • Assess health knowledge and learning abilities of patient and family; assesses the educational needs of the wound care and ostomy patients and families and develops appropriate plans for teaching

  • Utilize all available resources and consults as necessary with members of the health care team in planning and developing individualized teaching and discharge plans

  • Timely completion of all necessary paperwork

  • Function as a knowledgeable resource person to the staff in areas of expertise, related to ostomy care, pressure sore prevention and treatment, and management of the incontinent patient

  • After identifying staff learning needs, provide the necessary staff education

  • Participate as preceptor in the orientation of new staff and/or students as assigned by supervisor

  • Serve as a resource person to clinics, physician's offices, and businesses in the areas of expertise

  • Ability to comply with the productivity standards

  • Work closely with Home Services Materials Management, Home Services Medical Equipment, and outside vendors as needed to coordinate availability of needed equipment, and to improve quality of service to patients

Qualifications:

Required qualifications for this position includes:

  • Bachelor's Degree Graduated from an accredited school of Nursing

  • Coursework/Training: Completion of an accredited course in Wound, Ostomy and Continence

  • Current and unencumbered registered nurse license in the State of Oregon.

  • Staff that provide service in the State of Washington require a Washington State nursing license

  • Certification as a Enterostomal Therapist by the Wound Ostomy and Continence Nursing Certification Board within 3 months of hire

  • In compliance with the Providence Health & Services-OR Human Resources policy #302 Background Checks, a criminal background check is required for this position, based on assigned duties

  • Valid driver's license

  • Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire

  • Current BLS (Basic Life Support for Health Care Provider) or ACLS (Advanced Cardiac Life Support) required within 30 days of hire

  • Two (2) years Nursing experience

Preferred qualifications for this position includes:

  • One (1) year Home care experience (either prior to or upon certification as ET)
