REGISTERED RESPIRATORY THERAPIST

SITE OF SERVICE:

  • Naval Medical Center, Portsmouth, VA.

  • Ancillary services shall be provided in support of Naval Medical Center Portsmouth, VA (NMCP) and its associated Branch Health/Medical Clinics (BHC) and Tricare Prime Clinics (TPC).

    POSITION QUALIFICATION/REQUIREMENTS:

  • Degree: Associates Degree.

  • Education : Graduate from a program accredited by the Commission on Accreditation for Respiratory Care (CoARC) or Commission on Accreditation of Allied Health Education Programs (CAAHEP).

  • Experience : Possess at least 1 year of experience within the preceding 3 years as a Certified or Registered Respiratory Therapist in either (a) Pediatric Inpatient unit OR (b) Pediatric Clinic OR (c) Adult Intensive Care Unit.

  • Certifications :

  • Possess certification as Certified Respiratory Therapist (CRT) by the National Board for Respiratory Care (NBRC).

  • Possess and maintain certification in Pediatric Advanced Life Support (PALS).

  • Possess and maintain certification in Advance Cardiac Life Support (ACLS).

  • Possess and maintain certification in Neonatal Respiratory Program (NRP).

  • The employee shall hold current Healthcare Provider (HCP) Basic Life Support (BLS) certification using American Heart Association guidelines.

  • U.S. Citizenship : HCWs performing under this contract shall be U.S. citizens.

  • English Language Requirement : The Contractor shall ensure that all HCWs providing services under this contract are able to read, write, and speak English well enough to effectively communicate.

  • HCW Physical Capability : HCWs shall be physically capable of standing and/or sitting for extended periods of time and physically capable of performing all services required.

    SPECIFIC DUTIES:

  • The HCW shall provide services within the scope of their competencies, the applicable duties provided in the basic contract, and the following:

  • Efficiently administer all types of respiratory care, and efficiently draw blood, in accordance with professional qualifications, and as directed by the Respiratory Therapist (Advanced) or physician.

  • Operate and troubleshoot all types of respiratory equipment to include proper disassembly, cleaning, sterilization, and packaging of all respiratory therapy equipment to ensure safe patient care.

  • Maintain equipment, to include performing examinations to detect worn tubes, loose connections or other indications of disrepair and notify the Government supervisor of need for maintenance. Start equipment and observe gauges measuring pressure, rate of flow and continuity of test equipment. Notify the Government supervisor of malfunctions.

  • Be familiar with the functions and modes of equipment, ensuring equipment is cleaned, reassembled, and returned to storage.

  • Receive, set-up, operate, and maintain various devices and systems such as ventilators, nebulizers, oxygen therapy systems, humidity and aerosol therapy, sterilizers, and aeration chambers.

  • Administer prescribed doses of medical gases and aerosolized drugs intermittently and continuously. Perform airway management and hygiene including chest percussion postural drainage. Obtain arterial blood gas specimens.

  • Perform patient assessments and monitoring and provide patient instructions including breathing exercises. Make therapeutic recommendations. Examine patient records and report and identify changes in the clinical status of patients. Identify contraindications and report adverse responses/reactions.

  • Change nebulizers on nursing wards and reset oxygen flow as prescribed by physician.

  • Stock shelves in department and other departments of the MTF as needed for the provision of respiratory therapy.

  • Systematically monitor effectiveness of practice through client database and outcomes evaluation. Synthesize and use the results of these evaluations to recommend changes including policy, procedure or protocol documentation.

  • Initiate appropriate emergency treatment in cases of cardio-pulmonary arrest.

  • Assist in insertion of arterial catheters; perform drawing of arterial blood and complete measurement of arterial blood gas.

  • Collect cardiovascular and pulmonary artery pressure data from arterial catheters, measure cardiac output, do blood gas analysis and collect data from sophisticated computer modules applying trends recalled from these computers to follow patient status and prevent potential complications.

  • Assist physicians on consultation basis in choice of treatment according to goals set for individual patient.

  • Perform the most sophisticated and/or new pulmonary function testing, deriving standards, results, and interpretations from the feedback of a Technical computer system. Ability to interpret the above results.

  • Perform cardiopulmonary calculations in conjunction with shunt equations, cardiac outputs, ventilation/profusion quotients, Bohr equation, etc.

  • Provide advanced tracheostomy care such as tracheostomy changes, extubating, fenestration of endotracheal tubes, Olympic button placement, etc.

  • Provide set-up and monitoring of Nitric Oxide Administration in the intensive care units with patients receiving mechanical ventilation. Has the ability to change the tanks while a patient is receiving Nitric Oxide Administration

    GENERAL DUTIES:

  • Maintain a level of productivity comparable with that of other individuals performing similar services.

  • Participate in peer review and performance improvement activities.

  • Practice aseptic techniques as necessary. Comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection.

  • Function with an awareness and application of safety procedures.

  • Perform efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events. Anticipate potential problems/emergencies and make appropriate interventions. Notify supervisor, director, or other designated person regarding problems that the HCW is unable to manage.

  • Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner.

  • Participate in the implementation of the Family Advocacy Program as directed. Participation shall include, but not be limited to, appropriate medical examination, documentation, and reporting.

  • Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF.

  • Maintain an awareness of responsibility and accountability for own professional practice.

  • Participate in continuing education to meet own professional growth

  • Attend and/or comply with all annual training classes required by the Command, to include but not limited to online annual training provided by the MTF: disaster training, infection control, Sexual Harassment, Bloodborne Pathogens, Fire and Safety, Chemical, Biological, Radiological, Nuclear and Explosives (CBRNE), and all other required training.

  • Actively participate in the command’s Performance Improvement Program. Participate in meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist.

  • Participate in the provision of in-service training to clinic staff members. Provide training and/or direction as applicable to supporting government employe­es (e.g., hospital corpsmen, students, etc.).

  • Attend and participate in various meetings as directed.

  • Perform timely, accurate, and concise documentation of patient care.

  • Operate and manipulate automated systems such as CHCS, AHLTA, ADS, Essentris, MHS Genesis and Clinical Information System (CIS), participate in clinical staff Performance Improvement (PI) and Risk Management (RM) functions, as prescribed by the Commander. Maintain DOD email account as directed. The HCW shall be responsive to all email and voicemail communications.

  • Exercise appropriate delegation of tasks and duties in the direction and coordination of health care team members, patient care, and clinic activities

  • Maintain documentation of all treatment provided in accordance with clinic directives, and prepare such records and reports as may be required. All records and reports must be legible. Abbreviations must be only those listed in local instructions

    CORE DUTIES:

  • Perform complex respiratory procedures with minimal supervision.

  • Participate with physicians, nurses and other staff in planning respiratory treatment, with great reliance on the individual’s knowledge of the equipment.

  • Provide emergency or critical respiratory care.

    HOURS:

  • Respiratory Therapist (NICU/PICU/ICU) – The Respiratory Clinic is located in the main hospital and is operational twenty-four (24) hour a day. Services shall normally be provided Monday through Sunday. Typical shifts are as follows: Day shift 0630 – 1900, 0630 and 1500; Night Shift 1830 – 0700 and 2230 – 0700. Typically, there will be a need for two (2) HCWs on the day shifts and five (5) HCWs on the night shift. Each HCW shall normally provide services for either a twelve (12) or an eight and a half (8.5) hour shift to include an uncompensated thirty (30) minute meal break. Shifts will be on a rotated basis within the time ranges specified as clinical demand dictate. In no instance will the HCW be required to provide services in excess of eighty (80) hours per two-week period.

    SPECIAL REQUIREMENTS/SKILLS

    Must be comfortable in a fast-paced, dynamic environment. Must be able and willing to reprioritize on short notice and work on multiple simultaneous projects. Flexible and able to work with various personalities. Team work skills required. Time management skills required. The ability to meet deadlines in a deadline intensive environment is essential. High level of adaptability and willingness to embrace change in a fast-paced, demanding environment.
