Acts as a resource to theteam. Participates and leads quality improvement initiatives through coordinationwith leadership. Partners with the Manager of Assessment Referral Therapy/Outpatient. Ensures compliance with applicable regulatory guidelines andestablished departmental policies and procedures, objectives, qualityassurance program, safety, environmental and infection control standards. Coordinates orientation and training of new associates. Ensures staffcompetencies through proper training techniques and completes quality audits. Participates in problem solving to address issues relating to patientsafety, care and service. Supports and promotes effective working relationsand works effectively as part of the department/team, includinginterdepartmentally to facilitate the department's ability to achieve itsgoals and objectives. Collaborates with department leadership to coordinatecare with other departments and promote the highest level of patient care andsatisfaction. Assesses the patient's prior level of functioning, access to and/or useof community resources and available support systems. Assists the care team indeveloping a plan of care which includes, but is not limited to: assuringappropriateness of services and care setting, assuring individualizedsupport and education, determining the need for continued services,planning for discharge, and identifying and connecting patients/familieswith available community resources if needed. Collaborates with Physicians andother members of the health care team on the patient's behalf. Attends, facilitates and participates in rounds and case conferences.Advocates for patient rights.
